The new organization for the SOAP Note fields are:

• Medications

• Review of Systems (ROS)

Objective

• Vitals

• Physical Exams (PE)

• Photos of Exams

Assessment

• Diagnosis

• Notes

Plan

• Procedures

• Prescriptions

• Treatment Notes

• Patient Instructions

• Patient Handouts

Notes: The Plan/Patient Instructions and Uploaded Files are still the only fields that will reflect on the patient’s visit summary.
